Understanding Aged Care Home Care Packages

As our population ages, the need for care and support for older Australians has become increasingly important. The Australian government recognises this and has established the Aged Care Home Care Packages program to provide tailored assistance to elderly individuals, enabling them to continue living independently and comfortably in their homes. In this blog post, we'll delve into what these home care packages are, how they work, and the steps to access them.

An Aged Care Home Care Package is designed to offer a range of services and support to older individuals who require assistance to remain living in their own homes. These packages are tailored to individual needs and are categorised into four levels, from Level 1 (basic care needs) to Level 4 (high-level care needs). The goal is to empower older Australians to maintain their independence and enhance their quality of life while receiving the necessary care and support.

Aged Care Home Care Packages are available to:

  • People aged 65 or older (or 50 and older if you're an Indigenous Australian). In some cases, people under 65 who have specific care needs may also be considered.

  • Australian citizens, permanent residents, special category holders

Services Covered by Home Care Packages

The services included in a Home Care Package can vary based on the level of care and individual requirements. Some common services covered by these packages include:

  • Personal care: Assistance with grooming, bathing, dressing, and other daily activities.

  • Domestic assistance: Help with cleaning, laundry, meal preparation, and household tasks.

  • Allied health services: Access to physiotherapy, occupational therapy, and other allied health services.

  • Social support: Support to participate in community activities and maintain social connections.

  • Nursing services: Skilled nursing care, wound management, medication assistance, and more.

  • Home modifications: Adaptations to the home environment to improve safety and accessibility.

Steps to Access Aged Care Home Care Packages

  1. Assessment: The first step is to undergo an assessment by the Aged Care Assessment Team (ACAT/ACAS). They will evaluate your needs and determine the appropriate level of care required. You can contact My Aged Care (1800 200 422) to request an assessment.

  2. Eligibility: Based on the assessment, you'll be deemed eligible for a specific Home Care Package level. The higher the level, the more comprehensive the services.

  3. Waiting List: There might be a waiting period before a Home Care Package becomes available at your approved level. During this time, you can receive interim care services.

  4. Package Allocation: Once a Home Care Package becomes available, you'll receive a letter from My Aged Care notifying you of your package level and the funding amount.

  5. Choose a Provider: You have the freedom to choose an approved Home Care Package provider. Research and select a provider that aligns with your needs and preferences. If unsure, the team at Innovate Rehab will be more than happy to help you find an appropriate provider.

  6. Care Planning: Work with your chosen provider to develop a personalised care plan. This plan outlines the services you'll receive and how they'll be delivered.

  7. Service Delivery: Your chosen provider will begin delivering the services outlined in the care plan. Regular reviews will ensure the plan remains suitable.

If you think you may be eligible for a Home Care Package and need more assistance in accessing the scheme, or you have an approved Home Care Package and are seeking allied health providers, the team at Innovate Rehab will be more than happy to help you to achieve your goals, maintain your independence and enjoy a higher quality of life during your golden years.
