Accessing Innovate Rehab Services Through Medicare: What You Need to Know

At Innovate Rehab, we believe in making quality healthcare accessible to everyone. That's why we're proud to offer services under Medicare's Chronic Disease Management (CDM) Plan.


What is Medicare?

Medicare is Australia's national public health insurance program. It is designed to give Australians access to affordable, and sometimes free, healthcare services. It plays a key role in ensuring people can receive essential medical treatment without the financial burden.



How does the CDM Plan work?

If you've been living with a chronic condition (something that has been present for six months or more or is expected to last that long), you may be eligible for Medicare support through the CDM Plan. This plan was developed to help people manage long-term health issues by covering part of the costs for allied health services, such as those provided by Innovate Rehab.


Your general practitioner (GP) will be the one to assess your eligibility and, if appropriate, refer you for up to five allied health appointments per year. These appointments could include services like physiotherapy, exercise physiology, or occupational therapy, depending on what your healthcare needs are. Whether you need just one type of treatment or a combination, Innovate Rehab can tailor your plan to help you make the most of these sessions.

Once your GP has created your CDM plan, simply ask them to refer you to Innovate Rehab. We'll take care of the rest!
